When Is Revision Gynecomastia Surgery Necessary?
There are several common complications associated with gynecomastia surgery, which may lead a man to seek out revision surgery. Some of the most common reasons include:
- A reduction that was too conservative so that the male still has excess breast tissue remaining (most common complaint)
- Chest asymmetry
- Loose or sagging skin
- An overly aggressive resection that results in a depression or “crater deformity”.
- Lumpy appearance due to the formation of scar tissue beneath the skin
- Dissatisfaction with appearance of the nipples or areolas

Dr. Delgado’s Approach to Revision Gynecomastia
It is well known that revision gynecomastia is even more challenging than the initial breast reduction surgery. After any surgery, some amount of scar tissue will be present. This thicker, more fibrous tissue makes dissection more challenging and makes it harder to predict how the skin will contract after surgery. Since the skin is not likely to contract well, Dr. Delgado does not believe that liposuction alone is a sufficient solution to secondary gynecomastia surgery. He rejects many of the other popular approaches to gynecomastia surgery, including ultrasonic liposuction, because he has found these do not offer a favorable result.
In most cases, Dr. Delgado will use tumescent liposuction, as this offers him the greatest amount of control. When planning the surgery, he determines how to address and reduce scar tissue, as well as excess fatty and glandular tissue. When there are unnatural indentations, Dr. Delgado may decide to perform fat grafting or fat flaps to address these concerns. If the overall profile of the man’s chest is not appealing, he may perform additional sculpting to create a masculine contour.
